- Scan of passport (page with personal details),
- the orginal secondary education diploma or other orginal document equivalent to the Polish certificate of secondary education obtained abroad, legalized or bearing apostille,
- certificate stating that the issued diploma or other document entitles to apply for admission to study at universities of every type in the country where the institution issuing the certificate operates,
- and dokument confirming knowledge of English, i.e.: a certificate, diploma or other evidence of completion of secondary school abroad where classes were conducted in English or one of the dokuments proving the knowlege of English, Certicate of English TOEFL minimum score 510 (PBT) points in addition to at least 5.5 points(TWE), Certificate of English IELTS- minimum score of 6.0.
